At the Nourishing Wellness Medical Center, our goal is to provide the highest level of
personalized integrative medical care. Participation in your own care is vital to your success. Following these instructions and guidelines will ensure Dr. Peters and his staff are able to provide the best care possible.
Becoming a client
Becoming a client at Nourishing Wellness is simple, we can assess a program that will best suit your health needs. For your convenience, you can download many of our forms and policies, listed below.
Prior to your office visit, please download and review the policies and complete the forms. Fax or mail them back to the Nourishing Wellness office prior to the day of your appointment. This will enhance the time you have together. It will allow the Nourishing Wellness Team time to help solve your problems more efficiently. A few minutes spent ahead of time will add to the quality of care at the time of our appointment.

MEDICAL RECORDS
If this is your first consultation with Dr. Peters or his staff, it is imperative to have your medical records on hand prior to your appointment. This will ensure that they can provide you with the best possible comprehensive care. If you have any new tests or consultations with other health care providers after your care has begun, please send those by mail or fax to Cassi, our front desk manager, prior to your next appointment.
Please call Cassi at 310-373-7830 with any questions . She can assist you with the request from your physician to have your records be sent to our office. Medical records can only be released with your authorization. Cassi will fax or mail you an "authorization to release protected health information" for you to use with your other health care providers.
CANCELLATION POLICY
Due to our growing requests for consultations, a 24-hour cancellation policy has been implemented; otherwise you will be charged for the full amount of the consultation. You may cancel by calling the office at (310) 373-7830 If you call after hours, leave a message and your appointment will be cancelled.
CONSULTATION FEES
- Initial consultation with Allen Peters, MD: $250
- Initial consultation with Jeanne Peters, RD: $150
PAYMENT & INSURANCE INFORMATION
We are a cash-based center. You may pay by check or credit card. At your request, we will give you a "super bill" to submit to your insurance company for reimbursement.
COORDINATION WITH PHYSICIANS OR OTHER HEALTH CARE PROVIDERS
Often Dr. Peters is asked to confer with your primary care physician or specialists you may have seen. He will be happy to send copies of records, tests and treatments plans to your other physicians. By request, he can call your physician, or a three-way conference call can be arranged. His primary concern is to ensure that all your medical care is coordinated and integrated with all your health care providers.
LABORATORY TESTING: AN INNOVATIVE APPROACH
Have you ever felt sick enough to be examined by your doctor, have blood tests done, and x-rays taken, only for to have your doctor report back that, "All your tests are normal"? Yet both you and your doctor know you are sick?
Unfortunately this experience is all too common. Most physicians just order the same familiar lab test and x-rays. The usual tests may not look for deep enough for the etiology of your symptoms .
The usual tests do not look for the causes of illness. The test offered through Nourishing Wellness Medical Center offer detailed lipid analysis, nutritional deficiencies, metabolic imbalances, food allergies and neurotransmitter levels and more.
Dr. Peters uses these cutting edge diagnostic tests to help his patients prevent illness, and discover the cause for many chronic and difficult to treat conditions. Some testing can be done through conventional laboratories, and others only through specialty laboratories.
Dr. Peters will guide you through the best assessment to help you prevent and reverse disease. Some test can be performed at home. Others may require you to go to a local laboratory to draw the blood. In all cases Dr. Peters can order and coordinate all initial and follow up testing.
You will receive copies of all your test results and be guided through their interpretation and implications for treatment through follow up consultations with Dr. Peters
